Fill out the waiver using your legal first and last name as it appears on your license or ID. YOU MUST BE 18 YEARS OR OLDER TO FILL OUT THE WAIVER

INSTRUCTIONS FOR PARENTS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Child's information will be entered later

IMPORTANT!
1) Waivers for minors must be signed by a PARENT (or COURT APPOINTED legal guardian).
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
